What are examples of chronic conditions?

A chronic condition is a health condition or disease that is persistent or long-lasting in its effects or a disease that develops with time. The term chronic is often applied when the condition lasts three months or more.

Angel Perez, MD, PA, provides exceptional care for the following chronic conditions but not limited:

  • Asthma
  • Arthritis
  • Cardiovascular Disease
  • Depression
  • Diabetes
  • High Blood pressure
  • High Cholesterol
  • Hyperlipidemia
  • Thyroid problems
  • and more

Can a person be screened for their risk for chronic conditions?

Angel Perez, MD, PA, offers tests and screenings for chronic conditions. Specialized screenings include but not limeted:

Diabetic Retinopathy Eye Screening

Your Diabetic Retinopathy Eye Screening is performed using a handheld camera. For this screening, your eyes  most of the time don’t need to be dilated.

You may not need treatment immediately in the early stages of diabetic retinopathy. If left undiagnosed, diabetic retinopathy can progress and require more invasive treatments, including medicated injections to prevent fluid buildup in your eye.

Why are screenings important for chronic conditions?

Internists and primary care providers (PCPs) go into their field primarily to help others. While most know that they can’t treat everything themselves, they don’t want patients to leave without understanding whatever medical issues they may be managing.

For example, if you have signs of diabetes-related eye disease, your provider will refer you to a specialist for additional care. They can also customize a care plan to control your diabetes and other chronic conditions.

If your eyes are healthy, your provider can determine how often you need a diabetic eye screening or other tests based on your overall health and the severity of your diabetes and other chronic conditions, like thyroid disease.

If you manage one or more chronic conditions, call the office of Angel Perez, MD, PA, to schedule a diabetic eye screening and other testing, or request an appointment online today.
